Adult Health Assessment
As part of the Health Development Project, we are inviting adults living with spina bifida and/or hydrocephalus in Ireland to take part in our Adult Health Assessment. The Adult Health Assessment aims to help individuals monitor their own health whilst providing them with a baseline for future health comparisons. Additionally, the data from this assessment will be instrumental in enabling SBHI to facilitate a greater standard of understanding and experience with regards to positive health promotion in the adult spina bifida and hydrocephalus community in Ireland.
️To take part in the Adult Health Assessment, you must be over 18 years of age. Please note - the assessment is optional. The Adult Health Assessment is a detailed survey and may take approximately 45 minutes to complete. It is also important to note that the final page of the Adult Health Assessment offers each individual the opportunity to list the areas of their care which require follow-up, and express a desire to give more information on any particular area of living with these conditions. Your answers can also be downloaded as a pdf directly following your submission.
If you decide to take part in the Adult Health Assessment, you will be re-assessed in 2021 and 2022 to identify any changes in your healthcare. When we receive completed assessments, each adult will be given a unique identification number which will allow us to remove names.
